Older adults, 60 and older, living in Archuleta, Delores, La Plata, Montezuma and San Juan counties are eligible for Always Well services. Eligible caregivers may be younger than 60 as long as the person they provide care for is 60 or older.

 

Some programs have a waitlist and therefore, those with lower incomes and greater social need may be prioritized.
